Associate Wellbeing Coordinator (CareLine) - Contract

In this contract role, you will collaborate with Primary Care Providers, Community Nurses, Wellbeing Coordinators, and other healthcare professionals to deliver comprehensive care and support services to individuals and families in the community. Based in the vibrant city of Denpasar, Bali, you will be part of a team working to improve the health and wellbeing of the local population through proactive and preventive care strategies.

As an Associate Wellbeing Coordinator, you will play a crucial role in the CareLine program, focusing on health promotion, disease prevention, and health education. You will work closely with Primary Care Providers to assess and address the health needs of the community, develop and implement health programs, and provide support and guidance to individuals and families. Your daily tasks will involve conducting health screenings, providing health education sessions, and coordinating with various stakeholders to ensure the delivery of high-quality care.

📋 Tanggung Jawab Pekerjaan

• Collaborate with Primary Care Providers, Community Nurses, and Wellbeing Coordinators to deliver comprehensive care and support services to individuals and families in the community.
• Conduct health screenings and assessments to identify and address the health needs of the community.
• Develop and implement health programs and initiatives aimed at promoting health, preventing disease, and improving the overall wellbeing of the community.
• Provide health education sessions and workshops to individuals and families, covering topics such as nutrition, exercise, chronic disease management, and mental health.
• Coordinate with local healthcare providers, community organizations, and government agencies to ensure the delivery of integrated and holistic care.
• Monitor and evaluate the effectiveness of health programs and initiatives, and make recommendations for improvements.
• Maintain accurate and up-to-date records of health assessments, screenings, and program activities.
• Participate in team meetings and contribute to the development of strategies and plans for the CareLine program.

🎓 Kualifikasi & Syarat

• Bachelor's degree in Public Health, Nursing, or a related field.
• Minimum of 2 years of experience in community health, public health, or a related field.
• Certification in Community Health, Public Health, or a related field is a plus.
• Strong knowledge of health promotion, disease prevention, and health education principles and practices.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse individuals and groups.
• Ability to work independently and as part of a team in a fast-paced and dynamic environment.
• Proficiency in using Microsoft Office Suite, including Word, Excel, and PowerPoint.
• Familiarity with health information systems and electronic health records is a plus.
